What forms do I require?

Required Forms are listed below:

• Declaration Form 1 - PDF
  This form should be read carefully as it explains the grant process in detail.  Please follow this.  Failure to complete this form in full may delay processing of claim.
• Grant Claim 14 - R&D Facility (Capital Expenditure) 
• Auditors Certificate Template - Technology Capability
• Current Tax Clearance Certificate (if not previously provided).  This Certificate is provided by the Revenue Commissioners and can be applied for on their website www.revenue.ie or your Local Revenue Division.
