What forms do I require?

Required Forms (including Competitiveness Fund and Productivity Improvement Fund) are listed below:

• Declaration Form 1 
  This form should be read carefully as it explains the grant process in detail.  Please follow this.  Failure to complete this form in full may delay processing of claim.
• Claim Form 2 - Site and Buildings- Site & Buildings (including modifications)
• Claim Form 3 - Plant & Machinery- Plant & Machinery 
• Auditor's Certificate Template
• Environmental Profile Form (if not previously submitted)
• Current Tax Clearance Certificate (if not previously provided).  This Certificate is provided by the Revenue Commissioners and can be applied for on their website www.revenue.ie or your Local Revenue Division.

Where can I send my application?

Initially completed forms can be returned by e-mail to Grant Administration.  However, signed hard copies will be required to complete the payment process.
