What forms do I require?

Required Forms are listed below:

• Declaration Form 1 
  This form should be read carefully as it explains the grant process in detail.  Please follow this.  Failure to complete this form in full may delay processing of claim.
• Claim Form 8 - Rent Subsidy
• Auditor Certificate Template - Rent Subsidy
• Current Tax Clearance Certificate (if not previously provided).  This Certificate is provided by the Revenue Commissioners and can be applied for on their website www.revenue.ie or your Local Revenue Division.

Initially completed forms can be returned by e-mail (click on the icon below) to Grant Administration.  However, signed hard copies will be required to complete the payment process.
