What forms do I require?

Required Forms are listed below:

  • Declaration Form 1 
    This form should be read carefully as it explains the grant process in detail.  Please follow this.  Failure to complete this form in full may delay processing of claim.
  • Mentor_Grant_Claim_Form
  • Current Tax Clearance Certificate (if company has been grant aided over €10,000 within a twelve month period).  This Certificate is provided by the Revenue Commissioners and can be applied for on their website www.revenue.ie or your Local Revenue Division.
